- Small for gestational age: Up to 0.48 mg/kg/week.
- Chronic renal insufficiency: 0.045-0.050 mg/kg/day (or 1.4 mg/m²/day).
Adult Dosage
- Weight-based GHD start: Not more than 0.04 mg/kg/week, increasing to max 0.08 mg/kg/week every 4-8 weeks.
- Non-weight-based GHD start: 0.15-0.3 mg/day (or ~0.2 mg/day), titrated up by 0.1-0.2 mg/day every 1-2 months to max ~1 mg/day.
- Women or those over 60 may need adjusted higher/lower doses based on response.
Contraindications
- Acute critical illness post-surgery, trauma, or respiratory failure, where mortality rose significantly (42% vs. 19% placebo).
- Active malignancy or recent cancer diagnosis.
- Closed epiphyses in children, diabetic retinopathy, or hypersensitivity to somatropin/metacresol.
- Prader-Willi syndrome (PWS) patients with severe obesity, sleep apnea, or untreated respiratory issues, due to reported fatalities.
Major Warnings
- Intracranial hypertension (benign, papilledema, vision changes); discontinue if suspected, higher risk in Turner/PWS.
