Description

Genotropin (somatropin) 12 mg (36 IU) in a 3 ml cartridge is a recombinant human growth hormone used primarily for treating growth failure in children and adults with growth hormone deficiency.

Key Indications
In pediatric patients, it’s indicated for growth failure due to inadequate endogenous growth hormone, Prader-Willi syndrome, Turner syndrome, or being small for gestational age without catch-up growth. For adults, it replaces growth hormone in those with deficiency from pituitary/hypothalamic issues, surgery, radiation, or trauma.

Dosage

Genotropin (somatropin) 12 mg dosage varies by age, condition, weight, and response, always requiring physician supervision with IGF-1 monitoring. Weekly doses are divided into 6-7 subcutaneous injections.

Pediatric Dosage

• Growth hormone deficiency (GHD): Up to 0.47 mg/kg/week.
• Prader-Willi syndrome: 0.24 mg/kg/week.
• Turner syndrome: 0.33 mg/kg/week.
• Small for gestational age: Up to 0.48 mg/kg/week.
• Chronic renal insufficiency: 0.045-0.050 mg/kg/day (or 1.4 mg/m²/day).

Adult Dosage

• Weight-based GHD start: Not more than 0.04 mg/kg/week, increasing to max 0.08 mg/kg/week every 4-8 weeks.
• Non-weight-based GHD start: 0.15-0.3 mg/day (or ~0.2 mg/day), titrated up by 0.1-0.2 mg/day every 1-2 months to max ~1 mg/day.
• Women or those over 60 may need adjusted higher/lower doses based on response.

Contraindications

• Acute critical illness post-surgery, trauma, or respiratory failure, where mortality rose significantly (42% vs. 19% placebo).
• Active malignancy or recent cancer diagnosis.
• Closed epiphyses in children, diabetic retinopathy, or hypersensitivity to somatropin/metacresol.
• Prader-Willi syndrome (PWS) patients with severe obesity, sleep apnea, or untreated respiratory issues, due to reported fatalities.

Major Warnings

• Intracranial hypertension (benign, papilledema, vision changes); discontinue if suspected, higher risk in Turner/PWS.
• Neoplasms: Monitor for progression/recurrence; screen for intracranial tumors before starting.
• PWS-specific: Evaluate airway/sleep apnea pre-treatment; interrupt if snoring worsens; ensure weight control.
• Turner syndrome: Increased otitis media, cardiovascular risks (stroke, aortic issues).
• Fluid retention in adults (edema, arthralgia, carpal tunnel); dose-dependent and transient
