Topiramate should generally be avoided during pregnancy due to the risks of congenital malformations and neurodevelopmental disorders in the baby:

• Multiple studies have shown a 2- to 5-fold increased risk of oral clefts (cleft lip and palate) in infants after maternal use of topiramate early in pregnancy. The risk is more pronounced at higher doses used to treat epilepsy.
• Children prenatally exposed to topiramate have a higher chance of autism spectrum disorder (ASD) or intellectual disability compared to the general population. Around 3 in 100 exposed children may be diagnosed with ASD, compared to 1 in 100 in the background population.
• Topiramate can also cause fetal growth restriction when used during pregnancy.

Dosage

Monotherapy for Epilepsy

• The recommended maintenance dose for adults and children 10 years and older is 400 mg/day in two divided doses, titrated over 6 weeks 
• For children 2-9 years, the dose is weight-based, starting at 25 mg/day and titrated up to a maximum of 400 mg/day 

Adjunctive Therapy for Epilepsy

• The recommended dose for adults is 200-400 mg/day in two divided doses, titrated over several weeks 
• For children 2-16 years, the dose is approximately 5-9 mg/kg/day in two divided doses, titrated over several weeks 

Migraine Prevention

• The recommended dose for adults and children 12 years and older is 100 mg/day in two divided doses, titrated over 4 weeks

• Store Topamax 25 mg tablets at room temperature, 59°F to 86°F (15°C to 30°C)
