Tambocor 100mg tablets contain the active ingredient flecainide acetate and are used to treat various heart rhythm disorders (arrhythmias) such as severe symptomatic supraventricular arrhythmia, AV nodal reciprocating tachycardia, arrhythmias associated with Wolff-Parkinson-White syndrome, paroxysmal atrial fibrillation, and sustained ventricular tachycardia

Mechanism of Action

Flecainide is a class IC antiarrhythmic agent that works by blocking sodium channels in the heart, slowing down electrical impulses and prolonging the refractory period. This helps prevent abnormal heart rhythms from starting or spreading.

Contraindications

Tambocor is contraindicated in patients with:

• Second- or third-degree AV block (unless a pacemaker is present)
• Severe sinus node dysfunction
• Cardiogenic shock
• Uncontrolled heart failure
• Significant widening of the QRS complex
• Severe renal impairment

Adverse Effects

Common side effects of flecainide include:

• Visual disturbances (blurred vision, double vision)
• Dizziness
• Headache
• Nausea
• Constipation

Serious adverse effects like proarrhythmia can occur, especially in patients with structural heart disease. Close ECG monitoring is required.

Pharmacokinetics

Flecainide is well absorbed orally with a bioavailability of 90-95%. It is metabolized in the liver and has a half-life of 12-27 hours. Steady state plasma levels are reached in 3-5 days.

Interactions

Flecainide levels can be increased by CYP2D6 inhibitors like quinidine, amiodarone, and SSRIs. Rifampicin and other CYP2D6 inducers may decrease flecainide levels.   Dosage

  • The usual starting dose for adults is 50-100mg every 12 hours
